Before you begin your application….

You will be asked to confirm acceptance of the scheme requirements. Please do not complete the form unless you are authorised to do so.

The following will be required to be uploaded during the application process. Please make sure they are to hand before you start:

1. Photo for ID card – this should be a photo of your head and upper body, saved as a picture file (GIF, JPG or PNG) and not PDF as this is not compatible with the resizing software.

2. Public Liability Insurance (NOT Employers Liability Insurance) This must show:

· Name of business insured (to match name on application and name on Companies House, if applicable)

· Scope of cover (e.g. heating engineer or similar)

· Policy start and end dates (must have at least one month remaining)

· Amount of cover (minimum £2 million/€2.6 million)

3. Certificate(s) of achievement - We require a copy of the certificate of achievement for every scope you are applying for*. NB we cannot accept the candidate result sheet or an email from the training centre

*A certificate must be uploaded for each box ticked. If you have a combined certificate you will need to upload it again for each scope.

PLEASE NOTE:

Registration for any scope apart from liquid fuel is subject to an initial evaluation before you can become certified. Liquid fuel registration is subject to an initial three-month probationary period, and the initial evaluation will take place during this period.

For solid fuel registration, entry is acceptable via the OFTEC 108 training course, or if you are applying with a HETAS appliance installation course, this is also acceptable, but only if accompanied by the HETAS H006 System Chimneys training certificate.

CPS Registration Form

OFTEC will process your data in accordance with the General Data Protection Regulation (GDPR). We only collect your data for the purposes of granting certification with the OFTEC registration schemes. We will process and hold your data lawfully, fairly and securely. Your data will be processed accurately and kept up to date either by you notifying us of any changes, or via your own updates to your online registration account.

As part of the agreement to provide your business and technician(s) with registration, OFTEC will keep you updated on any industry changes that affect the work you do. Receiving these updates counts towards your commitment to obtain industry-related professional development to enable continuous registration. We will also notify you on any matters affecting your registration and will also send you reminders before your qualifications or registration expires. These may be by post, email and/or telephone.

In addition to providing your certification with the OFTEC registration schemes, OFTEC may introduce new or discounted products and services for heating professionals, or offer other related services provided by third parties. To choose whether to receive these offers or not, please indicate your preferences below. Please note, you can change these preferences at any time.

Section 4 - Application declaration

The registration agreement expects the work you carry out under your certified scope(s) of registration to meet the standards required by OFTEC in its published technical books, subsequent amendments and other legislative documents. You are also expected to comply with the rules of the OFTEC registration schemes (Document R100) as published in its current version.

I certify that, to the best of my knowledge, all the information I have given in this form is correct and that I agree to the conditions listed above. I understand that any inaccurate statement or omissions may disqualify me from registration with OFTEC.

OFTEC negotiates discounts and benefits for registered technicians with carefully selected third parties. This includes products such as insurance and discounted fuel cards. Please confirm whether you would like to receive information about these offers.
